I received a file that has an .exe extension. Do I still need a free mime application/x ms download to open the file?

If you weren't expecting to receive a file that has an .exe extension, then you don't want to open it, irrespective of whether you need a handler for mime-type application/x-ms-download or not.  Unsolicited .exe files are generally trojan droppers.

No. There is no need for a free mime application/x ms download to open an .exe file. This file types are executable files that automatically opens when you select them.
